In Halbrite, Saskatchewan on November 29, 1989, Mr. Frank VanDeWeyer entered a crude oil tank to free a maintenance worker trapped inside. Responding to the neighbouring trucking company's call for help, Mr. VanDeWeyer strapped on a breathing apparatus, entered the tank and found the victim lying unconscious, two baffles from the entry hatch. He began pulling the worker toward the hatch but the victim became lodged in the small passage adjacent to the opening. At that time Mr. VanDeWeyer was forced to exit the tank for a fresh air-supply unit. Other help arrived to dislodge the victim and transport him to the hospital where he was pronounced dead on arrival.
